Symptoms

On : 8.57 version, Change Assistant, SQL Server,  DB2, Second environment Change Package was applied to

When attempting to apply change package, the following error occurs.

ERROR
-----------------------
Msg 2705, Level 16, State 4, Server SERVER1, Line 1
Column names in each table must be unique. Column name 'REPORT_OPT' in table 'PS_AP_APY1099_AET' is specified more than once.

 
The issue can be reproduced at will with the following steps:
1. Created a change package
2. Applied initial Pass on one of the environment.
3. Attempt to apply the same script to the next environment in Move to Production (MTP) mode.
