EPY: Direct Deposit Search Page Has Multiple Last Name Searches

Symptoms
The Request Direct Deposit Search Page has multiple last name searches for "Second Last Name".
There should only be one field called "Second Last Name" on the search page for Request Direct Deposit not two,
The issue can be reproduced at will with the following steps:
1. Payroll for North America>Employee Pay Data USA>Request Direct Deposit
2. Search page will show two "Second Last Name"
