Symptoms
In PeopleSoft Enterprise Human Capital Management (HCM) 9.2, ACA files transmitted to the IRS failed with the following error:

XML Schema Validation Failed - not well formed or missing required elements
STEPS
-----------------------
1. Create ACA XML files for transmission to the IRS (Navigation: Benefits > ACA Annual Processing > Transmittal and Forms > Create ACA XML Files).
2. Submit the files to the IRS.
