Symptoms

Dental plan type 11 enrollments are not being included in the HIPAA 834 5010 flat file output, unexpectedly.

Plan type 11 enrollments are missing from submissions to a recent dental plan provider

STEPS
-----------------------
1. refresh the Ben100a results
2. create a HIPAA 5010 app engine run control
3. run the App Engine process
4. check the results
