Getting error when completing onboarding for Employee self service - W-4 Tax Withholding
Customer sees error: Username not defined in database (158,55)
STEPS to REPRODUCE

The issue can be reproduced at will with the following steps:
1. Begin Onboarding for new Employee
2. Choose W-4 PDF
3. Download W-4 PDF
4. Update W-4 PDF
5. Click Submit
6. See error "Username not defined in database (158,55)"
