What settings are required for the EOFM Service and Service Operations(security, handlers, routings, etc..) to use Forms and Approval Builder?

Background : When no service operations are active, user receive errors requiring EOFM_MENU and EOFM_LIST to be activated.
When activating the handler and local routing for Service Operation EOFM_MENU,  user gets duplicate listings when pressing the 'Fill out a new form' button.
