ELM - Survey With Required Field Giving Data Integrity Error (Doc ID 2597092.1)

Symptoms

From a Survey with required values and if click submit button without filling in value or selecting an option, receive an error message but then immediately get a data integrity error.
 

ERROR

Data structure from Page does not match the Data structure from the Database.

(Trace has an error message: Deserialization error: missing frame or stream SBlockData)

Steps to Replicate Issue:
1. Log in as a Learner and enroll in a class with a survey
2. For a question do not provide a value or select an option and click submit button
3. Receive an error message.
