Cannot Add Attachments or View Existing Attachments on Requisitions, Purchase Orders (POs) And Contracts

Symptoms
Cannot upload attachments and get below error when trying to view existing attachments on Requisitions, Purchase Orders (POs) and Contracts.
The issue of trying to upload attachments shows errors in the App Server Log. The issue of not being able to view attachments gives an error message:
Attachment failed to download. (18036,10014) File not found. (18036,10012)
Steps to reproduce the issue:
1) For Contracts: Procurement Contracts>Add/Update Contracts – Find Existing Value
2)Search for a contract
3) Click on Edit Comments Link in the header section
4) Displays the Contract Entry – Header Comments page. Verify there is an attachment. Click on the “View” button.
5) Message is displayed: Attachment failed to download. (18036,10014) File not found. (18036,10012)
6) Click on the Add button to get a new comments section. Click on Attach button. Click on browse to find a file to upload.
7) Select the file, and click on Upload.
8) Briefly get the spinning circle, like it’s working on attaching the file. Spinning circle disappears, and no file is attached. The document name is not there.
NOTE: The same thing happens when a new Contract is created and attaching a file.
Requisitions
1) Navigate to eProcurement>Manage Requisitions – search for existing requisition
2) Click on “edit more comments and attachments.”
3) Header Comments page displays. Verify there is an attachment. Click on View.
4) Get the message: Attachment failed to download. (18036,10014) File not found. (18036,10012)
5) Click OK to return to requisition
6) Add a line attachment (new).
7) Click on Add Attachments, browse for file and click on Upload.
8) It looks like it’s working to add the attachment, but it does not attach. Nothing has been attached and no file name.
9) Create a new requisition and add try to add a header and line attachments and get the same
10) Issue as above – no file is attached.
Purchase Order
1) Purchasing>Purchase Orders>Add/Update POs
2) Search for an existing PO.
3) Click on Edit Comments in the PO header section. Verify attachment file is there, and click on View.
4) Get the message: Attachment failed to download. (18036,10014) File not found. (18036,10012)
5) Add a new comment, and try to upload an attachment. After finding a file and clicking upload, no attachment is uploaded and no file name.
6) Same this happens when I create a new PO and try to attach a file to header or line.

EvalAddAttachment (error): return code (3) indicates attempt to upload file to storage failed.
Attempt to view file shows below error:
EvalViewAttachment (error): return code (9) indicates attempt to view file in storage failed.
