Symptoms
When attempting to add a new Benefit Plan to a Benefit Program, the following error occurs:

Plan Type X, Option Id X can only use a Rate Table defined with a Rate-per-Unit of 'None'. (3000,138)
The Health (1X) Plan Types have no concept of a "coverage amount". Therefore, they cannot use a Rate Table that
defines the rate as "Per-Hundred" or "Per-Thousand". Only a Rate Table defined with a Rate-per-Unit of "None" (a flat
rate) amount can be used here.

The issue can be reproduced at will with the following steps:
1. Add a new Benefit Plan to a Benefit Program and click Save (Navigation: Set Up HCM > Product Related > Base Benefits > Program Structure > Benefit Program Table, Plan Type and Option and Cost pages).
