Benefits Billing Enrollment Page Does Not Perform as Expected when Manually Enrolling Employee in Billing if Employee Has Ben Enrolled Previously (Doc ID 2623478.1)

Symptoms

An employee has previously been enrolled in Benefits Billing but that record is now inactive. A new row is inserted to begin a new period of billing and the Status is changed to Active, but the "Percent Calculation" and "Rate Percent" boxes remain grayed out.The user needs to scroll to the previous row first and then scroll back to get the boxes ungrayed or click Save (which action fails) then go back and select one of the boxes at that time.

Steps to reproduce the issue:

     1. A new row is added to enroll an employee in Benefits Billing who has had a previous period of enrollment (Navigation: Benefits > Benefits Billing > Manage Acct Status and Balance > Enroll in Billing).
     2. Change the Status to Active.
     3. Percent Calculation and Flat Amount boxes are grayed out.
