Symptoms

When using a screen reader to apply for a job, it is difficult to determine which question each "I decline to answer" checkbox is associated with without the visual context on the Diversity step of the job application.  Both checkboxes have the same name and are announced the same.

When using quick navigation tools like the VoiceOver rotor of NVDA's elements list, these checkboxes are not distinguishable.

It may also be difficult for a user using voice control to accurately select the correct checkbox.

This is failure of SC 2.4.6 because the labels are not descriptive enough to clearly distinguish each checkbox.
