ETL9.2: Approve Reported Time Performance Issue

On : 9.2 version, Approvals:
We have a performance issue on timecard approval via manager dashboard or manager self service. The page just keeps wheeling until timeout. No changes to system.When looked at the app server logs found follwoing SQL error on some users.

Also attached the full app server log.
We tried to delete app server cache and rec'd this error (see attachment cache deletion error).
The issue can be reproduced at will with the following steps:
1. Manager approves time from Manager self service or dashboard > approve reported time
2. There are 5 managers approving time at same time
3. Each manager does not have more than 15 employees to approve time for at same time
