Symptoms
When trying to click on Add Attachments in My Forms Error message observed.

The URL catalog entry specified in the first parameter of ViewURL() was not found.
The issue can be reproduced at will with the following steps:
1. Navigate Fluid ESS and click on My Forms
2. Click on Fill on New Form
3. Fill in the information on the form and Click on Save button
4. Click on Add Attachment button at the bottom of the page.
5. Error observed.
