ETL9.2: Missing Approval Buttons On Fluid MSS Enter Time Page

Symptoms
The approval buttons are not showing on the Fluid Enter Time page as they used to. If using accessibility mode, they show up on the very bottom of the page.
The issue can be reproduced at will with the following steps:
1. Enter time for a punch time reporter that needs approval.
2. Log in as a manager and view that employee's timesheet via Team Time > Enter Time page.
