Symptoms
Error message received when trying to fill out a new Form
ERROR
The component FORM_ADD has not been registered to enable supplemental data.
The issue can be reproduced with the following steps:
1. Navigate to My Forms
2. Click on Fill out a new form
3. Error observed.
