Why the Fields Configured or Added on Fluid Design a Form are Not Visible? (Doc ID 2770280.1)

Trying to utilize the Fluid Forms and Approvals Builder for the first time, configured and activated a form but when a 'user' attempts to fill out the form, none of the fields configured or added are visible.

Is there any setting that needs to be enabled for the fields to show up on the form?
