E-PORTAL: Pressing the Home Button After Accessing "Change my password" Page from a Tile Throws: "Error Getting Portal &PortalURI=http:."

Symptoms
PortalURI parameter is not getting populated correctly after pressing the Home button when returning from "Change my password" page when accessed from a tile.
It gets generated without <hostname>:<port>, causing the following error:

The issue can be reproduced at will with the following steps:
1. Login in PIA and navigate to root > Change my password
2. Add this page on a Homepage
3. Go and access the newly created "Change my password" tile
4. Click on the Home icon
5. Receive a white page with the following error: That was not a valid URL format
i.e. link: http://<hostname>:<port>/psc/<site_name>/&PortalURI=http://s/WEBLIB_PTBR.ISCRIPT1.FieldFormula.IScript_StartPage
