"Fatal Peoplecode SQL Error" and "Error while updating Response records with new Version key" Occurs When the user Atempts to Approve an Event With More than One Version

Symptoms
Fatal Peoplecode SQL Error Occurs when the user tries to Approve an Event with more than one version from Approval tile.
Steps to reproduce the issue:
1. Navigate : Sourcing > Create Events > Event Details
- Create an Event
- Enter Line Item
- Invite Public Bidders
- Click Post
- Event Status : Pending Post Approval
2. Login as Approver
- Select the Event
- Click Approve
- There are no errors
3. Login back to the source creator
- Navigate : Sourcing > Maintain Events > Event Workbench
- Select the Event
- Create new version
- Select Yes
- Update Event Name
- Click Post
- Event Status : Pending Post Approval
4. Login as Approver again and go to Approver tile
- Select the event
- Click Approve
- Receive below Error
