E-PIA: Blank Page Displayed When Adding New Vaccination Information

Symptoms
When adding the vaccination information from the Vaccinations tile, users are getting a blank page. Screen hangs up and users have to refresh the page in order to exit.
The issue can be reproduced at will with the following steps:
1. On the Employee Self Service homepage, select the Health & Safety tile
2. Click Vaccinations tile
3. Click Add Vaccination (+) button
4. Enter in data and click Continue
5. A blank page is displayed
