Template Add Button is not Active When Adding a New Personal Template that has Template Name Different Than the Template Description

Template Add button is not working and not active when adding a new personal template that has template name different than the Template description on fluid requisition page.
The issue can be reproduced at will with the following steps:
- Navigate to eProcurement Tile > Create requisition tile > and create a new requisition.
- Search for any item or add a special request item.
- Add this item to template, enter template Name different than the template description.
- Now press on the template tab and notice that the Add button is not active.
