Student Advisor Assignment Process Assigns More Than One Advisor in Some Cases

Symptoms
When running the Student Advisor Assignment process using the same assignment code but on different days, the process assigns more than one advisor to students. The process should assign only one advisor.
The issue can be reproduced at will with the following steps:
- Update Instructor/Advisor table for Advisor 1 with Academic Career, Program and Plan. Navigation: Records and Enrollment => Student Background Information => Student Advisor.
- Update Advisor 2 with the same values.
- Go to Student Assignment page and create a new code. Navigation: Records and Enrollment => Student Background Information => Student Advisor Assignment.
- Enter required parameters, including Distribute Advisors by:Single and Assign Student Advisors at: Program/Plan.
- Click 'Get Advisors'. Both Advisors display as expected.
- In Self-Service => Student Center, update Student 1 with above Program and Plan.
- Go back to Student Advisor Assignment and enter a newly created Assignment Code. Navigation: Records and Enrollment => Student Background Information => Student Advisor Assignment.
- Press 'Get Students': Student 1 was added as expected.
- Press 'Assign Advisors' button.
- Check Student 1 assignment: Advisor 1 was assigned as expected.
- On the next calendar day, in Self-Service => Student Center, update Student 2 with the same Program and Plan.
- Go back to Student Advisement and using the same Assignment Code press 'Get Students' button.
- Student 2 was added as expected.
- Click 'Assign Advisors' button.
- Check results for both students.
- Got expected results for Student 2.
- Results for Student 1: first Effective dated row shows only one advisor but the second Effective dated row shows that both advisors were assigned to the student.
