Symptoms

The HIPAA 834 File does not include terminations but should.

Steps to reproduce the issue:

     1. Run the Refresh Benefit Snapshot process (Navigation: Benefits > Interface with Providers > Refresh benefit Snapshot).
     2. Run the Create HIPAA 834 file (Navigation: Benefits > Interface with Providers > Create HIPAA 834 File).
