Benefit Dependent Attachment First Denied Appears Again For Approval When a New Uploaded Document Needs To Be Approved

An employee uploads a document to qualify a new dependent. That document was reviewed by an approver and denied. The employee then uploads a new document. When the approver again views documents to be approved, the pending approval screen shows both the new and the previously denied documents.
Steps to reproduce the issue:
1. Employee uploads a document requiring approval to add a new dependent (Navigation: Employee Self Service homepage > Dependent/Beneficiary tile > link to add Document for dependent)
2. Approver denies the document (Navigation: Manager Self Service homepage > Approvals tile).
3. The employee uploads a new document (Navigation: Employee Self Service homepage > Dependent/Beneficiary tile > link to add Document for dependent).
4. Approver sees both the new and the previously denied documents on the pending approval screen (Navigation: Manager Self Service homepage > Approvals tile).
