Symptoms

When trying the solution on "The Component FORM_ADD Has Not Been Registered To Enable Supplemental Data" Error Message Observed In 'My Forms' (Doc ID 2713211.1), customer is unable to choose the data under the lookup: there are no base records to choose from the prompt.

The issue can be reproduced with the following steps:
1. Navigate to My Forms
2. Click on Fill out a new form
3. Error observed.
