eProcurement Requisition Randomly Getting Auto Approved with no Entry in AWE Tables

Symptoms
eProcurement requisitions are getting marked as approved randomly, but there is no entry in the XREF (PV_REQ_AW) table or any other AWE tables like - EOAW_STEPINST, EOAW_USERINST etc.
Steps to Reproduce the Issue:
1. Create Fluid ePro Requisition
Navigate to Self Service > eProcurement > Create requisition
Add an item and quantity that should trigger the approval workflow.
2. Press Checkout and Submit.
As result, the requisition is approved without being trigger to approval workflow.
