Look Up Reason Window Displays 'No matching values were found.', After Clicking Reason Field Prompt on Create Class Block and Block Enrollment Merge Pages in Add Mode

Symptoms
On the Create Class Block and Block Enrollment Merge Pages, when attempting to add a new value, clicking the magnifying glass prompt for the Reason field results in the Look Up Reason window displaying ''No matching values were found.', instead of the expected valid Enrollment Action Reason values.
The action reason can be applied to each individual student and class combination by first merging then retrieving the student and class blocks, but this more labor intensive approach should not be necessary.
The issue can be reproduced with the following steps:
- Navigate to Records and Enrollment > Enroll Students > Block Enrollment > Create Class Block.
- Click 'Add' to add a new value.
- Enter Institution and Class Enrollment Block.
- Select Term and Class Nbr.
- Select the magnifying glass for “Reason” and note the resulting lookup window displays "“No matching results were found.”
- Navigate to Records and Enrollment > Enroll Students > Block Enrollment > Block Enrollment Merge.
- Click 'Add' to add a new value.
- Select Institution, then Select Student Block or use Detail/Create.
- Select Detail/Create for Class Block, and on the Create Customized Class Block page, enter term and class number.
- Select the magnifying glass for “Reason” and note the resulting lookup window displays "“No matching results were found.”
