Symptoms
When running Create W-2 PDF Forms, Tax960US.sqr fails with the following errors in the log file

(SQR 3506) Array specified not defined with CREATE-ARRAY.

(SQR 3506) Array specified not defined with CREATE-ARRAY.
Steps to Reproduce
1. Run the Load process for year end (Payroll for North America > U.S. Annual Processing > Create W-2 Data > Load Year End Data)
2. Run Create W-2 PDF Forms (Payroll for North America > U.S. Annual Processing > W-2 Reporting > Create W-2 PDF Forms)
3. Error occurs
