Questionnaire Framework Notification Email Issues: It Can Be Completed Multiple Times and Only Contains the Oprid of the User id Who Created the Questionnaire

Symptoms
Using the Questionnaire Event, an email is sent out with a link to the questionnaire. Although the option "Allow Users to Update after Save" was set to "No" during questionnaire setup, it can be opened and completed multiple times which is not correct.
The email is sent to all employees who have were chosen via the recipient list functionality. But the link from all the emails includes only the Oprid of the user id who created the questionnaire and it seems no other users (than the one who created the questionnaire) can access the page.
They receive the following error:
Steps to replicate the issue:
1. Use the Questionnaire Category page (EOQF_CATEGORY_FL) to create questionnaire categories (Enterprise Components > Questionnaire Framework > Questionnaire Categories).
2. Use the Question Pool page (EOQF_QUESTION_FL) to add questions to a pool which can later be used when creating a questionnaire (Enterprise Components > Questionnaire Framework > Questionnaire Question Pool).
3. Use the Answer Pool page (EOQF_ANSWER_FL) to add answers which can be later added to the questionnaire (Enterprise Components > Questionnaire Framework > Questionnaire Answer Pool).
4. Use the Manage Questionnaires page to create new questionnaires (Enterprise Components > Questionnaire Framework > Manage Questionnaires).
5. Use the Recipient List page (EOQF_RECIPIENT_FL) to add or edit a recipient list for questionnaire event (Enterprise Components > Questionnaire Framework > Questionnaire Event > Questionnaire Event Setup > Recipient List > View Recipient List > Select Add button > Recipient List).
6. Use the Notification Template page (EOQF_NOTIF_FL) to add or edit notification message text for questionnaire events (Enterprise Components > Questionnaire Framework > Questionnaire Event > Questionnaire Event Setup > Notification Template > View Notification Template > Select Add button > Notification Template).
7. Create Questionnaire Event page (EOQF_EVENT_ADD_SCF) to create a new questionnaire event and define basic event information (Enterprise Components > Questionnaire Framework > Questionnaire Event > Manage Questionnaire Events > Manage Questionnaire Events > Select Create Event).
8. Use the Adhoc Notification Execution page (EOQF_EVENT_RUNCNTL) to run an adhoc notification process for a specific questionnaire event. AE “EOQF_RUNEVNT” ends in success.
9. Result: All employees who were chosen via the recipient list functionality receive an email.
But the link from all the emails includes only the OPRID of the user id who created the questionnaire which is not correct.
Due to this, when any employee (beside the one who created the questionnaire) click on the URL link, following error occurs. "You don't have access to this page".
