Symptoms
Forms grid does not display the sequence number. This is used as an identifier for business function.

Steps to replicate:
1. Click My Forms tile.
2. Create a new Form, populate needed details and save, or, access an existing one.
3. Open the filled form, Seq Nbr (Sequence Number) is not displayed.
