TAM: When Generating an Application Report (HRS_BIP_APPL) the Process Is Not Printing the Questionnaire Type Section

Symptoms
When generating an Application Report (HRS_BIP_APPL) the process is not printing the Questionnaire Type section.
The issue can be reproduced at will with the following steps:
1. Login as a Talent Acquisition Manager. (Or Recruiting Manager)
2. Navigate to Recruiting > Search Applications
3. In the Search Applications page, clear all the default values and select "071 Offer Accepted" in the Disposition drop down. Click Search
4. From the Search Results, click on the Print icon of Application for any Applicant
5. In the Application Report Options page, check Preferences, Education Level and Additional Questions check boxes
6. Scroll all the way down and click on Generate Report.
