Symptoms
There are some additional segments that are required per the government document. The segments are in the Transaction Set Header section, and they are Segment ID's REF01, REF02, DTP01, DTP02, DTP03 and INS13. According to the Government 834 File layout requirements doc, these segments are flagged as "Situational" however there is a note(3) in the REF section that seems to supersede that status, it states the following; "This segment is REQUIRED when the contract or trading partner agreement identify a Policy Number for use with electronic enrollment".
