E-SEC: App Server Message "Signon PeopleCode was not executed"

Signon PeopleCode is not executing, and this may be causing a number of issues with Password Controls. Users with expired passwords are able to log into the application with LDAP Authentication.
The error as seen in the application server log is this:
PSAPPSRV.12247 (2863) [05/13/08 13:30:50 GetCertificate](3) Error Setting Sign on PeopleCode context for User USER123@abc.com: Sign on PeopleCode was not executed
