Shipping Address not Populated when creating Orders from Account Summary and the Applet "Order Entry - Payment Form Applet"

Information in this document applies to any platform.
There is two issues here:
1) The shipping address is not getting populated when the Order is created from the Account Summary view and applet "Order Entry - Payment Form Applet".
2) As a result of first issue it seems that some line items are populating with the address of the account even though the shipping address is empty.
