Open UI Blank Screen, Blank Page or Aurora Side Menu Not Visible in Environment Upgraded to Innovation Pack 2014 or IP15 or IP16 (Doc ID 1963536.1)

Symptoms

ACTUAL BEHAVIOR

After upgrading from 8.1.1.11 (IP13) to 8.1.1.14 (IP14) or IP15 or IP16, logging into the OpenUI application renders a blank screen/page and no further navigation is possible.
This issue is applicable for thin / dedicated / mobile clients. It has also been noticed that the Aurora side menu bar is not visible.
