Siebel 8.1.x Outbound Communication Manager Is Sending Multiple Outbound Emails Intermittently and creating Duplicate activities (Doc ID 2000682.1)

We are using BS: Outbound Communications Manager in the workflow to send email and sometimes it is sending multiple mail and returning multiple activity Ids as we can see it int he Workflow which in debug level. Attaching the screen shot "Outbound_2 Activities".

Also this is intermittent and not able to reproduce the issue.

Symptoms

On : 8.1.1.11.16 [IP2013] version, Comms Server & Email Response

Customer is using BS: Outbound Communications Manager in the workflow to send email and sometimes it is sending multiple mail and returning multiple activity Ids.

In the log:

CommSrvr CommSrvrTrace 3 00000006552469e0:0 2015-04-08 10:19:53 0: Sent message to , "(SR#10309486) has been retrieved from you by Test1E".
CommSrvr CommSrvrTrace 3 000000455524134c:0 2015-04-08 10:19:53 1: Sent message to , "(SR#10309486) has been retrieved from you by Test1E".
...
CommSrvr CommSrvrTrace 3 000000465524134c:0 2015-04-08 10:19:54 Sent 2 out of 2 messages  (2.000) seconds.

 
Also this is intermittent and not able to reproduce the issue.

BS: Outbound Communications Manager should not send multiple mail and returning multiple activity Ids.

The issue can be reproduced at will with the following steps:
1. Send email outbound using BS: Outbound Communications Manager
2. Sometimes it is sending multiple mail and returning multiple activity Ids.
3. This is intermittent
