CTI Toolbar Not Enabled In OpenUI or High Interactivity Mode After Upgrade, A Blank Space Show Up Where The CTI Toolbar Should Appear.

On Siebel 188.8.131.52 OpenUI mode when attempting to use CTI Toolbar , it is not loaded and the following error occurs in Browser Developer Console:
:ERROR: commToolbarpui.js: No place holder for CTI toolbar!
:ERROR: commToolbarpui.js: Init process failed!
In the Siebel Application Object Manager log corresponding to the user session the following error can also be observed:
ObjMgrSRFLog Error 1 00000031564b00d0:0 2015-11-17 20:09:27 (objdef.cpp (15931)) SBL-DAT-00259: Menu Item 'Consultative Transfer' and Menu Item 'Accept Work Item' in Menu 'Consultative Transfer' both specify the same position.
ObjMgrSRFLog Error 1 00012b3354db4a2c:0 2015-02-12 13:49:43 (objdef.cpp (13326)) SBL-DAT-00259: Menu Item 'Media Indicator' and Menu Item 'Agent State For Work Item' in Menu 'Media Indicator' both specify the same position.
ObjMgrSRFLog Error 1 0000016356550748:0 2015-11-25 15:18:22 (objdef.cpp (15931)) SBL-DAT-00260: There is no menu item definition at position '16' specified as the parent for menu item 'Accept Email' in menu 'Communication'.
