ACTUAL BEHAVIOR

Check box is not displayed properly when query on a applet.

EXPECTED BEHAVIOR
------------------------------
Check box should be displayed properly at the time of query on the applet.

STEPS
----------
1. Login into the application.
2. Navigate to the any Form Applet with check boxes and click on Query button.

BUSINESS IMPACT
------------------------
UI issue. Check box not displaying properly when query on the applet.
