CTI Phone Nunber (Extension) Has Not Been Saved , even after explicitly clicking 'Save' Button and Re-login.

We are encountering siebel system access CTI voice platform issue.
when user login in, they can change extension number from 88138 to 88111 under 'User Profile -> Communication'. After pressing save button, the record is saved successfully . But when user log out and then login in again, the extension number went back to 88138, the field under Communication went back to 88138 again. logs files are attached, Please check and help us to analyze the issue.
