Diagnostics Screen Not Showing All The Details

We have recently upgraded our production environment to and are monitoring AOM and other comp crashes in the system.
While we can see the core dumps, fdr and callstacks, we are not able to see the corresponding information in the diagnostics screen in Administration-Server Management screen properly. Also, for whatever failed processed it has logged in that screen, not all fields are populated. For ex: Failure Time field is blank for all the failed processes.

How can we fix diagnostics screen to show all the details?
