Siebel Reports Failing Intermittently with Error "Template not found" (Doc ID 2107423.1)

Symptoms

After the upgrade reports are failing intermittently with the error:

SBL-EAI-04308: Operation 'runReport' of Web Service 'http://xmlns.oracle.com/oxp/service/v11/PublicReportService.PublicReportServiceService' at port 'PublicReportService_v11' failed with the following explanation: "oracle.apps.xdo.webservice.exception.OperationFailedException: PublicReportService::generateReport failed: due to oracle.apps.xdo.servlet.data.DataException: Template not found: IBM_Service_ServiceHistory_V4.rtf".

For the failed reports, there is a reference to a previous version of the templates in all the errors and the error is Template not found: IBM_Service_ServiceHistory_V4.rtf".
