'Response Log QA' Records Not Created, When Landing Page Survey Answered Using Trackable URL (Doc ID 2122510.1)

Symptoms

On : 15.7 [IP2015] version, Siebel Marketing

'Response Log QA' records are not created, when landing page survey answered using Trackable URL

Expectation :
========
Response record is created and child records in 'Response Log QA' are created.

Actual Result:
=========
Only response is created (S_COMMUNICATION) table.
Noticed that records got created in Response Log Stager ('S_LEADRESP_IMPT') table.
but 'Response Log QA' records are not created. (S_COMM_SVY_LOG).

Noticed error : "SBL-DAT-00498: 'First Name' is a required field. Please enter a value for the field. in log file" attached

STEPS
-----------------------
The issue can be reproduced at will with the following steps:
1. Create web survey
2. Click on trackable URL
3. Check 'Response Log QA' records

BUSINESS IMPACT
-----------------------
Records not created in 'S_COMM_SVY_LOG' table as expected to store the 'Question Name', 'Answer' values of response.
