Workflow Policy Trigger Multiple Times For Same Policy Creating Duplicate Records Or Execute Workflow Policy Action Multiple Times For Same Record in S_ESCL_REQ

Workflow Monitor Agent component (alias WorkMon) or custom clone of WorkMon component is triggering multiple time for the same Workflow Policy record in S_ESC_REQ, consequently the associated Workflow Policy Actions on Workflow Policy Group trigger multiple time for the same S_ESCL_REQ record causing unexpected behaviors. Some scenario observed:
“SC Workflow Monitor Agent SWI” a cloned Workflow Monitoring Agent component is monitoring Workflow Policy Group “SWI Customer Update Policy Grp” which has Workflow Policy Action that invokes workflow "SWI Contact Update Workflow". This standard workflow sends contact/account/address related updates to UCM via AIA.
Workflow Policy has been triggered multiple times instead of only once. A custom workflow policy 'Create Assets' has been triggered three times instead of one in production environment. This workflow will use a business service to create asset in Account > Asset, as it has been trigger multiple time for the same record i S_ESCL_REQ duplicate Assets have been created.
