OpenUI: User Logs In and Gets Blank Page After Applying Siebel Patchset 15.16

On : 15.13 [IP2015] version, Installation
When attempting to log on to the Application Object Manager a blank screen appears after login. This started happening after applying patch 15.16 on 15.13.
The issue cannot be reproduced with sample database nor on internal environment; it could only be reproduced using customer modified database even using standard SRF.
The issue can be avoided if the old require-ext.js file used from previous patchset 13.
The issue can be reproduced at will with the following steps:
1. Open a Browser
2. Enter URL for the Siebel application
The issue has the following business impact:
Due to this issue, users cannot use Siebel.
