Issue With Lead To Opty Conversion - Primary Contact & Account Is Not Getting Set

On : 16.0 [IP2016] version, Client Functionality
While associated a prospect to a Lead and when doing Lead to Opty conversion, even though the Prospect is getting converted to corresponding account and contact records, the Primary Contact in the Opty is not getting set and also newly converted account is not getting associated with the Opty.
While associated a prospect to a Lead and when doing Lead to Opty conversion, the Primary Contact in the Opty should set and also newly converted account should get associated with the Opty.
The issue can be reproduced at will with the following steps:
1. Login to FINS Application
2. Go to Prospect Screen
3. Create new Prospect (Give First Name and Last Name and Account)
4)Go to Leads Screen
5)Create new Lead and associate Newly created Prospect to the Lead
6)Click on Convert Button in Lead (Before clicking on Convert Button,Please activate 'Promote Prospect (Single)' and 'Lead To Opty Conversion' workflows from Sitemap-->Administration-Business Process-->Workflow Deployment screen)
7)User Navigates to 'Campaign Lead' view.
8)Check Account Field is populated with Account or not in Opportunity applet
9)Check Contact associated to Opportunity is set as Primary or not in contact list applet
The issue has the following business impact:
Due to this issue, Primary Contact in the Opty is not getting set and also newly converted account is not getting associated with the Opty.
