Records Are Not Loading Into IIR 9.0.1 Tables (Doc ID 2227086.1)

Symptoms

After upgrading IIR version to 9.0.1 SP3 and followed the steps to configured Siebel DQ for IIR, records are not loading to IIR tables.
Under IIR logs (ids.dbg) , error:

NOTE: "Specsavers_Customer" is specific in this case but can he substitute with any value as per customer architecture  


 In siebel Object Manager log, below errors:
