Radio Button Values Are Not Committed In DB When Multiple Records Are Present

Custom Radio Button Values are not committed in DB when multiple records are present.

For the first record the radio buttons are working fine, where as when the user selects 2nd record or any other record, and updates the radio button, the record doesn't gets committed.
