Fields Blanking Out When Creating An SR On An IPad

Siebel Mobile Specific - on version: 16.11 [IP2016]
Fields become blank after entering some values while creating SR record on an iPad device.
When the user creates a new Service Request on the iPad (using Siebel Service for Mobile App), some fields previously entered values disappear.
When the same application is tested using Chrome browser (on same iPad device), this issue is not replicated.
So it seems like it is something related directly to the iPad Mobile app.
The issue seems to only occur when there is a list applet at the top of the screen and the form applet below.
