CTI Driver Error

Symptoms
When attempting to use CTI toolbar we noticed that the toolbar sessions are getting logout very frequently. and CommSessionMgr tasks are getting piled up very often and the following errors occur:
ERROR:Failed to create service instance from GenCommDrv, channel-type-string is voice, connect-string is , parameters is
ERROR:Error on invoke media manager method(StartAllDriver), input args={ }, error=7537141), msg='Unable to load communications driver. Unable to create a Service object from driver GenCommDrv with channel string voice.(SBL-CSR-00501)'
ERROR:Failed to invoke method(StartAllDriver) at media manager, detail error = Unable to load communications driver. Unable to create a Service object from driver GenCommDrv with channel string voice.(SBL-CSR-00501)
ERROR:Communication Agent failed on prcoessing request(StartAllDriver0, input-args={
ERROR:Failed on invoking ClientConnect in Comm. Server Svc., err=7537141, msg=Unable to load communications driver.
The issue can be reproduced at will with the following steps:
1. Configure CTI
2. Make Outbound call
