Wrong Error Message When Duplicate Attachment/URL Records Are Created (Doc ID 2763449.1)

When creating 2 records with the same file or URL in any attachment applet, the following message is displayed:

An Attachment with this name already exists. Would you like to replace it?

But when 'OK' button is clicked, it does not replace the record but removes the new record. 'Cancel' button does the same.
