LDAP Suddenly Stopped Working, Error "SBL-SEC-10018: ldap_open failed"

Symptoms
The LDAP authentication suddenly quit working when logging into the application. There were no changes made to the LDAP or Siebel configuration.
Login fails and error seen in the OM log is:
SBL-SEC-10018: ldap_open failed
In checking into the behavior, ldapbind command was tested and seen to fail as well.
This command processes outside of the Siebel application layer and must work for Siebel to also be successful with LDAP connection.
